Over on DF, we have a thread about giants hiting, and whether armor/shields thus should be required to save vs crushing blow or not. Cause otherwise, the only time a CB save would get called for is the rare occasions someone smashes an item lying there WHILE under the effect of a giant str potion or the like, or for long falls...

So how do YOU handle it, when a giant (or ogre) thwaks a pc? Do they save each hit? On any hi the giant makes thats say above 18 natural? Only on nat 20s??

I go with the 1E DMG and require an item save vs. crushing blow on each hit from a giant. Not so much orges, because they don't seem tall that massive (9'-10' tall). I like that rule because it makes giants particularly dangerous and they're one of my favorite monsters (and they make the top ten list of monsters with most of my players).
